Clinical Analysis of Endoscopic Thyroidectomy by the Axillary Approach: A Report on 100 Cases

Abstract

|
Purpose: Thyroidectomy has recently become one of the newest fields for the application of endoscopic surgery. Still, some patients complain of even a small scar on the neck and anterior chest. Thus, we have preformed endoscopic thyroidectomy by the axillary approach, and this leaves no scar on the neck and anterior chest. In this study, we evaluated the effectiveness and safety of performing endoscopic thyroidectomy by the axillary approach.
Methods: We reviewed 100 patients who underwent endoscopic thyroidectomy for thyroid tumor between November 2001 and October 2003. During this period, endoscopic thyroidectomy was attempted for patients with benign tumor and micropapillary carcinoma, as was confirmed by fine-needle aspiration cytology. The patients with a history of irradiation on the neck or the patients who received previous neck surgery were excluded.
Results: We tried endoscopic thyroidectomy by the axillary approach for 106 patients. There were 6 cases of conversion to conventional thyroidectomy: 1 case of mechanical trouble, 2 cases of intraoperative bleeding and 3 cases of papillary cancer. The patients whose operations were successfully done were 99 women and 1 man with their ages ranging from 11 to 57 years (mean age: 36.1 years). The operation time were 133.19¡¾39.1 (range: 80¡295) minutes for unilateral thyroid tumor and 200.5¡¾55.86 (range: 100¡290) minutes for bilateral thyroid tumors. The post-operative hospital stay was 3.19¡¾1.19 (range: 2¡9) days. There were 91 cases and 9 cases of benign thyroid tumor and papillary carcinoma, respectively. The post-operative complications were 1 case of transient recurrent laryngeal nerve injury, 1 case of bleeding, 4 cases of minimal hematoma, 6 cases of paresthesia and 3 cases of temporary swallowing difficulties. All the patients were satisfied with the cosmetic results.
Conclusion: This operative procedure is safe and feasible with a good cosmetic effect. Endoscopic thyroidectomy by the axillary approach is an alternative method for conventional thyroid surgery.
